Follow these steps to complete the Online Enrollment process:
Step 1- Online
Complete the online enrollment forms by clicking on the link at the bottom of this page. Completion of the enrollment forms will take approximately 20-30 minutes. If you need assistance or have questions please contact Shelicia Brown, Family and Community Engagement Coordinator at 937-361-3175. Your call will be returned in a timely manner.
Step 2- Submit Required Documents to Finalize Enrollment
At the end of the enrollment application, you will have the option to upload required documents. These documents can also be sent via email to [email protected] as attachments with your child’s name listed in the subject line. Pictures of the required documents will also be accepted as long as the information is clear.
The following documents are required for enrollment and missing or incomplete documents will result in a delay of enrollment:
copy of birth certificate
(1) proof of residency- options include:
copy of parent/guardian ID or driver's license
court-stamped custody documents (if applicable)
IEP / MFE / 504 Plan (if applicable)
